Before or at the time of collecting personal information, we will identify the purposes for which information is being collected. We will only retain personal information as long as necessary for the fulfillment of those purposes.

We will collect and use personal information solely with the objective of fulfilling those purposes specified by us and for other compatible purposes, unless we obtain the consent of the individual concerned or as required by law (see below). We will protect personal information by reasonable security safeguards, as well as unauthorized access, disclosure, copying, use or modification.

We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned. For example, this website utilizes Google Analytics and server-based analytics packages. These gather limited information on visitors to our website automatically, and stores this anonymous information in server log files. It is used only for site administration, as well as for analysis of website usage and trends.

DATA WE COLLECT

Intake Form
Data submitted through the intake form is maintained on a dedicated private server and is only used to facilitate:

  1. the client’s use of health insurance benefits;
  2. the ability to communicate via phone, text, e-mail;
  3. a written record of HIPAA statutory compliance;
  4. a written record of client approval of the state mandated disclosure statement.

Contact Form
The “Contact Me” stores the content contained in each message sent. Each sent message is responded to as a business message via a separate email account neither located or controlled on this site. There is no other use made of the stored data. It can be deleted upon demand from the sender.

Financial Data
Located on another site is client data that is collected in person and real time via phone conversation. That data, specifically a valid credit or debit account, including an HSA account, facilitates clients’ ability to pay for health insurance cost shares, including deductibles, copayments and coinsurance payments, or private payments for service (subject to the terms of the state-mandated disclosure statement part of the intake form). The vendor ownership for that site is located at https://www.axiamed.com. seattle-counseling.com is solely responsible for providing any necessary audits or accountings to clients with inquiries.
